This particular aircraft crashed on 4 March 1981, after a mid-air collision with another F-4D, both operated by the 401st TFW, near Madrid, Spain.

It was repainted in USAF colourscheme and the serial 72-1282 was re-applied. Unfortunately, this aircraft was lost on 14 October 1998 in a mid-air collision with another German Phantom over the White Sands missile range.
