Destroyed in a mid-air collision with Bell 206B JetRanger N6TC while both were on sightseeing flights over the Grand Canyon on June 18, 1986. Previous regn was N386EX.

This Twin Otter first flew in November 1971 and worked with Wardair Canada's northern operation from January 1972 to December 1979. Sold to Ptarmigan Airways, who continued to fly it out of Yellowknife. The airline merged with Bradley Air Services (First Air) in January 1997 and although later sold to Arctic Sunwest / Summit Air, the Twin Otter continues to be based in Yellowknife.

Operated by the USAF Academy Parachute Team as a UV-18B. Serial 77-0464 is not worn.

After rolled out in early 1969, several sales of this Twin Otter to the USA fell through and the aircraft was delivered to Aeronaves de Mayab as XA-BOL December 19, 1969. West Coast Air Services bought it in September 1979, but it never entered service. It was sold to Contact Airways in April 1980 and photographed just before the titles were painted out and the tail logo became a red circle. While flying on floats with Maldivian Air Taxi as 8Q-MAG, the aircraft was written off (with no injuries) when it struck the water after a photo shoot at the Halaveli Island Resort, Maldives June 2, 2009.
