Crashed into power lines on approach to Prospect Bay, Rainy Lake, near Fort Frances, Ontario 5 October 2004. The pilot escaped and the aircraft sank in shallow water. The tail assembly and engine cowlings was noted at Jay Wendall (Whitey) Hostetler's Red Lake seaplane base during the same trip.

Previously operated by Nestor Falls Fly-in Outposts, Nestor Falls, Ontario, but as of Feb 2001 by Pipestone Air, Emo, Ontario. The aircraft was photographed approaching the seaplane facility on Rainy Lake , east of Fort Frances, Ontario at the end of Seven Mile Bridge, north of Highway 11. It was possibly heading to Lakeland Aviation for servicing. This aircraft crashed on approach near Prospect Bay, on the 5th of October 2004. The aircraft sank and the pilot escaped.
