It was wrecked on 14 June 1999 taking off from Thunder Bay, due to improper installation of the stabilizer pitch trim actuators. (To quote the report: "After getting airborne, the aircraft pitched up to approx. 70 degrees, reaching a height of about 500 feet. It then rolled to the left, pitched steeply nose-down, and descended to the ground within the confines of the airport." All occupants escaped injury from what must have been a terrifying experience.)
