Delivered to the U.S. Army with S/N 63-12923 in October 1964, and was in use by the Army in Vietnam from April 1965 through November 1968. Transferred to the U.S. Navy and given BuNo 312923. Assigned to Light Attack Helicopter Squadron 3 where it stayed until 29 February 1972 when it was returned to the U.S. Army. Returned to the U.S. and was transferred to Langley Research Center from October 1973 through August 1975, then in storage until 1994. Purchased surplus and restored to HA(L)-3 configuration, FAA reg. N370AS. Acquired by the HA(L)-3 Association in 2024 for display in the museum.
