25/01/1984 when assigned to VMFP-3 Det 1 Capt. Michael J. Healey and Maj. John M. Yencha were on a routine reconnaissance mission operating from the USS Midway (CVA-41) when the RF-4B experienced a utility hydraulic failure. The RF-4B made an emergency landing, but sustained extensive structural damage. For this display she was modified to look like a F-4J, before being placed at El Centro in 2000.

This RF-4B performed its first flight in June 1965 and was delivered to VMCJ-3 in August. This picture was taken shortly after that. Already five years later, the Phantom was sent into storage at MASDC, but in 1978 it went back into the air with VMFP-3. It was damaged beyond repair on 25 January 1984 on board USS Midway in a landing accident. The RF-4B is now preserved at El Centro.
