The display sign for this Phantom lists her as having been with the 334th TFS, 4th TFW at nearby Seymour Johnson AFB for her entire service history, however Baugher quotes a number of other units. Said sign also quotes this as being the last operational F-4E in service at that base. After leaving active service, this aircraft served as a BDR trainer at SJ AFB, and was later displayed there. She was placed on display at this location sometime in 2017 or 2018; it was opened and dedicated in April 2018.
