This is actually F-4C 63-7704 marked as F-4C 64-0571, the aircraft that Medal of Honor recipient Lance Sijan was flying when he was shot down over Vietnam on 9 November 1967. Lance was from Milwaukee, WI., which is why the memorial is located there. The actual F-4C on display, 63-7704, recorded a MiG-17 kill over Vietnam in 1967 while assigned to the 480th Tactical Fighter Squadron, 366th Tactical Fighter Wing.

This photo may have been taken at Misawa, the 356th TFS' homebase from 1968 until 1971. This F-4C shot down a MiG-17 in 1967, when operated by 480th TFS / 366th TFW at Da Nang AB. It is currently preserved in Wisconsin as "64-0751".
