Originally built as CH-47C 68-15992, delivered in 1969. Assigned to C Company, 159th Assault Support Helicopter Battalion in Vietnam. Loaned to Air America from late 1972 through mid 1974. Upgraded to CH-47D in 1991 and assigned new serial number of 90-0214. Served in Afghanistan and was damaged by a windstorm. Shipped back to the U.S. and was acquired by the museum in 2014.
