Written off 8 October 2019., it crashed into a wooded area near Zemmer, north of Trier (Germany). The pilot ejected, but was injured and taken to hospital. The F-16 with callsign "Warhawk 11" issued an emergency (squawk 7700) but it has been suggested that it could have been the wing man of the aircraft in trouble. The USAF Accident Investigation Board Report was released on 11 February, 2020. A combination of partail electrical power loss and poor weather immediately after take-off caused the pilot to eject and the US$ 25,551,000 F-16 came down 8 nautical miles south of Spangdahlem.

On 8 Oct'19, this F-16CM of 480th Fighter squadron crashed near the village of Zemmer Germany. The pilot ejected safely and was recovered with minor injuries. This photo was taken a few years earlier, when the Fighting Falcon participated in an exercise, it is equipped with inert AIM-120 AMRAAMs on the wing tips (stations 1 and 9), AIM-9X underneath the wing (2 and 8) and huge AGM-88 HARMs underneath the wings (3 and 7). The F-16 also carries, 370 gallon fuel tanks (4 and 6), a HARM targeting pod on the left intake (5L), a Sniper targeting pod on the right (5R) and an ALQ-184 ECM pod (5).
