This Phantom served the 86th Tactical Fighter Wing and was later on transferred to Fort Wayne assigned to the 163rd Tactical Fighter squadron as part of the Indiana Air National Guard. In October 1991 it was transferred to the Hellenic Air Force as 68-0394. While serving Greece, the aircraft (most probably assigned to 338 MPK) crashed on 8 August 1994.
