This Phantom served the 86th Tactical Fighter Wing and was later on transferred to Fort Wayne assigned to the 163rd Tactical Fighter squadron as part of the Indiana Air National Guard. In October 1991 it was transferred to the Hellenic Air Force as 68-0394. While serving Greece, the aircraft (most probably assigned to 338 MPK) crashed on 8 August 1994.

This Phantom served Hahn and Ramstein air bases (both Germany), and Fort Wayne air national guard base before it was sold in 1991 to Greece. The aircraft was reported as stored, after its Hellenic adventure, at Limnos (Greece) in late 2009. It's current fate is unknown.

The bands on the fin ar black/yellow/red with the 512th FIS emblem proudly presented. The band on the fuselage, as well as the horizontal flash are yellow. The squadron was based at Manston, Kent at the time and was part of the 406th FIW.
