Delivered to Midwest Helicopters in 1974, Forest Helicopters would acquire this helicopter in 1997 when they took over the northwest Ontario operations of Midwest Helicopters. C-GCDM was sold in 2010 to a numbered company operating as Essential Helicopters (now Helicopters Canada) offering training based at North Bay, ON.
