Rolled off the production line in 1964 and crash-landed in Missouri two days after the US Air Force received it. Although badly damaged, it was delivered to the Ogden Air Logistics Center at Hill Air Force Base for repair and hands-on training. It was the first aircraft to go through the F-4 Crash Damage Repair Program. To the Hill Heritage Museum in 1986.
