On May 14, 2022, the aircraft had departed North Perry for a flight to Key West. As it was flying over the Miami area, it experienced a loss of engine power. A landing was attempted on the Haulover Inlet Bridge, but the aircraft collided with an SUV, flipped over and caught fire. Unfortunately one occupants was fatally injured, 2 others onboard the aircraft were seriously injured. The driver and 2 small children in the SUV were not inured, however taken to the hospital as a precaution.
